Below is information on immunization requirements for your school age student.

If your student needs an immunization and he/she does not have private insurance or CHIP, your school nurse would be able to administer the required immunization (s).  The following forms may be downloaded, filled out and returned to your school nurse giving permission to adminster the required immunizations:

HPV is offered by the school nurse offices at the Jr. High and High School.  If your student does not have private insurance, and/or CHIPS, he/she would be eligible to receive the HPV vaccine.
